About The Position

Residential Home Health and Hospice is a nationally recognized leader in home health and hospice, and a consistent Top Workplace. We are hiring an Weekends Licensed Practical Nurse for a Hospice position providing end-of-life care to patients in their homes in the Mechanicsburg, PA area. This role involves delivering compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life. The position requires providing after-hours support by conducting ongoing patient assessments focused on comfort, symptom management, and changes in condition, escalating clinical concerns to the RN as appropriate. It also includes educating and supporting patients and caregivers during evenings, nights, and weekends by reinforcing end-of-life care plans, symptom relief strategies, and what to expect throughout the hospice journey. Thorough and compliant EMR documentation is essential. The role requires independent work, but with team support, and involves travel to patient homes within an assigned territory.

Requirements

• Active Licensed Practical Nurse (LPN) license in the state you work
• Two years of LPN experience and the clinical expertise to deliver quality patient care
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
