Hospice LPN/LVN
About The Position
The hospice licensed practical/vocational nurse (LPN/LVN) provides compassionate nursing care to help patients remain comfortable throughout their end-of-life journey. The nurse delivers skilled care according to individualized plans, administers medications and treatments, monitors patient responses, and communicates changes in patients’ conditions. The role also includes educating and supporting patients and families with medications, medical equipment, and care needs, while maintaining accurate and timely documentation. Depending on the assignment, care may be provided in patients’ homes, assisted living or skilled nursing facilities, or other hospice care settings. Some assignments may involve travel within an assigned service area, on-call responsibilities, or after-hours support for patients and families. Nurses work alongside their team to deliver compassionate, personalized care that honors each patient’s wishes.
Requirements
- Graduate of an approved practical or vocational nursing program.
- Current LPN/LVN license in the state where the nurse will practice.
- At least one year of nursing experience.
- Reliable transportation, a valid driver’s license, and required auto insurance for roles providing in-person care.

- Hospice, home health, hospital, or community health experience is preferred.
