About The Position

As a hospice licensed practical/vocational nurse (LPN/LVN), you will provide compassionate nursing care that helps patients remain comfortable throughout their end-of-life journey. You will provide skilled nursing care based on individualized plans of care; administer medications and treatments and monitor patient response; identify and communicate changes in a patient’s condition; educate and support patients and families with medications, medical equipment, and care needs; and maintain accurate, timely documentation. You will work alongside a team to deliver compassionate, personalized care with heartfelt empathy, honoring each patient’s wishes.

Depending on the assignment, care may be provided in patients’ homes, assisted living or skilled nursing facilities, or other hospice care settings. Some roles may involve travel within an assigned service area, on-call responsibilities, or after-hours patient and family support.

Requirements

• Graduate of an approved practical or vocational nursing program.
• Current LPN/LVN license in the state where you will practice.
• At least one year of nursing experience.
• Reliable transportation, a valid driver’s license, and required auto insurance for roles providing in-person care.
• Additional licensure may be required for certain telephonic or after-hours assignments.

• Hospice, home health, hospital, or community health experience is preferred.
