Hospice LPN (Licensed Practical Nurse) - Charleroi, PA
About The Position
Anova Hospice & Palliative Care is hiring a full-time Hospice LPN based out of our Charleroi office, serving patients across Westmoreland, Fayette, Washington, and Greene counties. This role involves Monday through Friday, daytime visits with no scheduled weekend shifts or nights, as dedicated on-call nurses handle those responsibilities. The position emphasizes paying for the entire day, including drive time between visits and documentation time, at the full hourly rate, along with mileage reimbursement. A sign-on bonus is also offered. The role involves 4-7 patient visits per day within a defined home territory, with consistent assignments under an RN case manager as part of an interdisciplinary team. The company is described as family-owned with local leadership.
Requirements
- Current Pennsylvania LPN license and CPR certification
- 3 years of nursing experience, including at least 1 year in hospice or palliative care
- Reliable vehicle, valid driver’s license, and auto insurance
- Strong organization and communication skills
- The steadiness to be a calm, skilled presence in hard moments
Responsibilities
- Provide skilled nursing visits for symptom management and comfort under the direction of the RN case manager and the plan of care.
- Administer medications per physician orders, including comfort kit medications.
- Perform wound care and other skilled interventions within LPN scope.
- Monitor and report changes in patient condition to the RN and physician.
- Reinforce teaching with patients and families on care techniques and what to expect.
- Document visits accurately and on time.
- Participate in interdisciplinary team meetings.
