About The Position

The Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) is responsible for providing direct patient care under the supervision of a Registered Nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Requirements

  • Current Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) licensure in state of practice.
  • Graduate of an accredited practical or vocational nursing program.
  • At least three (3) years of nursing experience in health care, hospice experience preferred.
  • Knowledge of and commitment to hospice philosophy of care.
  • Current CPR certification required.
  • Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

  • Provides direct patient care as defined in state’s Nurse Practice Act.
  • Implements current nursing practice following a comprehensive assessment and the plan of care.
  • Provides accurate and timely documentation consistent with the plan of care.
  • Assesses and provides patient, family, and caregiver education and information pertinent to diagnosis and plan of care.
  • Participates in coordination of hospice services, appropriately reporting the identified needs to the Interdisciplinary Group (IDG) and/or Director of Nursing.
  • Uses equipment and supplies effectively and efficiently.
  • Provides appropriate pain/symptom management and evaluates patient’s response to treatments and/or medications.
  • Participates in personal and professional growth and development.
  • Participates in quality assessment performance improvement teams and activities.
  • Performs other duties as assigned by the Registered Nurse.
