About The Position

The Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) is responsible for providing direct patient care under the supervision of a Registered Nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Requirements

Current Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) licensure in state of practice.
Graduate of an accredited practical or vocational nursing program.
At least three (3) years of nursing experience in health care, hospice experience preferred.
Knowledge of and commitment to hospice philosophy of care.
Current CPR certification required.
Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

Provides direct patient care as defined in state’s Nurse Practice Act.
Implements current nursing practice following a comprehensive assessment and the plan of care.
Provides accurate and timely documentation consistent with the plan of care.
Assesses and provides patient, family, and caregiver education and information pertinent to diagnosis and plan of care.
Participates in coordination of hospice services, appropriately reporting the identified needs to the Interdisciplinary Group (IDG) and/or Director of Nursing.
Uses equipment and supplies effectively and efficiently.
Provides appropriate pain/symptom management and evaluates patient’s response to treatments and/or medications.
Participates in personal and professional growth and development.
Participates in quality assessment performance improvement teams and activities.
Performs other duties as assigned by the Registered Nurse.
