About The Position

The Hospice LPN DON Assistant plays a critical role in supporting the Director of Nursing (DON) in delivering high-quality, compassionate care to patients in hospice settings. This position involves assisting with clinical oversight, ensuring compliance with healthcare regulations, and facilitating effective communication between nursing staff, patients, and families. The role requires a strong commitment to patient-centered care, focusing on comfort, dignity, and quality of life for terminally ill patients. Requirements

  • Licensed Practical Nurse (LPN) license in good standing within the state of practice.
  • Minimum of 2 years of clinical nursing experience, preferably in hospice or palliative care settings.
  • Minimum 2 years experience scheduling in Homecare Homebase
  • Basic knowledge of hospice care principles, symptom management, and end-of-life care.
  • Familiarity with healthcare regulations and documentation standards relevant to hospice care.
  • Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.

Nice To Haves

  • Experience working in a leadership or assistant leadership role within a nursing department.
  • Certification in Hospice and Palliative Nursing (CHPN) or related specialty certification.
  • Proficiency with electronic medical records (EMR) systems and healthcare software.
  • Training or coursework in quality improvement and regulatory compliance.
  • Ability to mentor and educate nursing staff and support continuous professional development.

Responsibilities

  • Assist the Director of Nursing in managing daily nursing operations and staff coordination within the hospice care team.
  • Provide direct patient care including medication administration, symptom management, and monitoring patient conditions.
  • Ensure compliance with hospice policies, state and federal regulations, and accreditation standards through regular audits and documentation review.
  • Support nursing staff by providing guidance, training, and assistance with clinical procedures and documentation.
  • Facilitate communication between patients, families, interdisciplinary team members, and external healthcare providers to promote coordinated care.
  • Participate in quality assurance and performance improvement activities to enhance patient care outcomes.
  • Maintain accurate and timely clinical records, including patient assessments, care plans, and progress notes.
