About The Position

The LVN Intake Coordinator serves as the primary clinical liaison for incoming hospice referrals and admissions. This position is responsible for coordinating the referral and intake process, facilitating timely patient admissions, verifying eligibility requirements, communicating with referral sources, and collaborating with the interdisciplinary team to ensure seamless transition of care. The LVN Intake Coordinator promotes the hospice philosophy while maintaining compliance with Medicare, state, federal, and agency regulations.

Requirements

  • Graduate of an accredited Vocational Nursing program.
  • Current and unrestricted Licensed Vocational Nurse (LVN) license in the state of practice.
  • Current CPR certification.
  • Valid driver's license and acceptable driving record.
  • Minimum one (1) year of nursing experience required.
  • Reliable transportation.
  • Current automobile liability insurance as required by agency policy.
  • Proficient in electronic medical records (EMR) and Microsoft Office applications.
  • Ability to maintain patient confidentiality and HIPAA compliance.

Nice To Haves

  • Hospice, home health, case management, intake, or admissions experience preferred.

Responsibilities

  • Receive, review, and process hospice referrals from physicians, hospitals, nursing facilities, assisted living facilities, and other referral sources.
  • Obtain and review clinical documentation to determine hospice eligibility.
  • Coordinate referral activities to ensure timely response and admission.
  • Verify physician orders and obtain all necessary documentation for admission.
  • Maintain accurate and complete referral records in the electronic medical record system.
  • Communicate with patients and families regarding hospice services, admission procedures, and agency offerings.
  • Address questions and concerns related to hospice care and services.
  • Coordinate admission scheduling with patients, families, referral sources, and hospice clinical staff.
  • Facilitate a smooth transition from hospital, facility, home health, or other levels of care into hospice services.
  • Collaborate with physicians, nurse practitioners, hospitals, discharge planners, social workers, and community partners to coordinate care.
  • Participate in interdisciplinary team communication regarding pending admissions and patient needs.
  • Communicate significant clinical findings to supervisory and clinical staff.
  • Assist in identifying urgent patient needs and prioritize admissions accordingly.
  • Ensure all referral and admission documentation meets Medicare Conditions of Participation and agency standards.
  • Maintain confidentiality of patient information according to HIPAA regulations.
  • Complete intake documentation accurately and within established timelines.
  • Participate in chart audits and quality improvement activities as assigned.
  • Support agency readiness for surveys, accreditation reviews, and regulatory audits.
  • Develop and maintain professional relationships with referral sources and healthcare partners.
  • Provide exceptional customer service to patients, families, referral sources, and staff.
  • Represent the agency in a professional manner consistent with hospice values and mission.
  • Promote the agency's philosophy, goals, and services within the community.
  • Participate in Quality Assessment and Performance Improvement (QAPI) activities.
  • Attend required meetings, in-service training, and continuing education programs.
  • Assist with monitoring referral trends and admission outcomes.
  • Contribute recommendations for process improvement and operational efficiency.
