About The Position
Concordia Hospice of Washington is seeking a dedicated and personable Hospice LPN to join their team. As a Home Health LPN, you will be a vital part of the patient's care team, providing skilled nursing care and support. This role involves administering medications and treatments, monitoring vital signs, assisting with wound care and personal care, educating patients and families, maintaining medical records, and collaborating with other healthcare professionals. You will also provide emotional support and companionship, building genuine connections with patients.
Requirements
- Strong clinical skills and knowledge of end-of-life care principles.
- Excellent communication, interpersonal, and organizational skills.
- Compassion, empathy, and emotional resilience to provide comfort and support during end-of-life situations.
- Ability to work independently and as part of a collaborative team.
