About The Position

The Hospice Licensed Practical Nurse (LPN) provides direct patient care to hospice patients under the supervision of a Registered Nurse (RN) or Physician. This role involves delivering compassionate end-of-life care, assisting with care plans, and supporting patients and their families during their final stages of life.

Requirements

  • Completion of an accredited Licensed Practical Nurse (LPN) program.
  • Current LPN licensure in the state of practice.
  • Must possess and maintain current CPR certification.
  • Strong clinical and patient care skills.
  • Excellent interpersonal and communication abilities.
  • Proficiency in electronic medical records (EMR) systems and office software.
  • Ability to work independently and as part of an interdisciplinary team.
  • Compassionate, empathetic, and dedicated to providing high-quality patient care.
  • Ability to lift up to 75 pounds and turn, move, and transport patients using proper body mechanics and lifting devices as needed.
  • Capacity to stand, walk, and sit for extended periods.
  • Ability to perform repetitive tasks and physical activities, such as bending, kneeling, and reaching.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Must be able to travel throughout the service area.

Nice To Haves

  • Previous experience in hospice or palliative care preferred.

Responsibilities

  • Administer medications, treatments, and procedures as prescribed by the physician and under the direction of the RN.
  • Monitor and record patient vital signs and report any changes in patient conditions to the RN or physician.
  • Assist patients with activities of daily living, including bathing, dressing, and grooming.
  • Follow individualized care plans developed by the interdisciplinary team to meet the physical, emotional, and spiritual needs of hospice patients.
  • Provide input to the RN and interdisciplinary team regarding patient care and progress.
  • Ensure that all care provided is documented accurately and promptly in the patient's medical record.
  • Provide emotional support and education to patients and their families regarding the hospice process and what to expect.
  • Communicate effectively with patients and families to address concerns and ensure they feel supported.
  • Assist families with the preparation for end-of-life care and decision-making.
  • Participate in interdisciplinary team meetings to discuss patient care plans and progress.
  • Collaborate with other team members, including nurses, physicians, social workers, and chaplains, to provide holistic care.
  • Coordinate with other healthcare providers to ensure continuity of care.
  • Follow infection control guidelines and maintain privacy rules and regulations as per HIPAA.
  • Attend all meetings and complete in-services and mandatory training.
  • Provide exceptional customer service and high-quality care by showing compassion and empathy towards patients and their families.
  • Performs other related duties as assigned.
