About The Position

The Hospice Intake Specialist plays a vital role in coordinating the referral and admission process for patients seeking hospice care. This position serves as a key point of contact for patients, families, and referral sources, ensuring a smooth, timely, and compassionate transition into hospice services. The Intake Specialist is responsible for gathering and verifying patient information, coordinating with clinical teams, and maintaining accurate documentation throughout the intake process.

Requirements

• High school diploma or equivalent required
• 1–2 years of experience in healthcare intake, hospice, home health, or a related setting required
• Strong organizational and multitasking abilities
• Excellent communication and interpersonal skills
• Proficiency with EMR systems and healthcare software
• Ability to manage sensitive information with professionalism and confidentiality

Nice To Haves

• Associate’s or Bachelor’s degree in healthcare or related field preferred
• Working knowledge of hospice services, insurance verification, and Medicare/Medicaid guidelines preferred

Responsibilities

• Receive and process hospice referrals from hospitals, providers, and families, ensuring all required documentation is complete and accurate.
• Communicate clearly and compassionately with patients and families regarding hospice services, eligibility, and the admission process.
• Accurately enter and maintain patient information in the EMR system, ensuring all records are current and compliant.
• Verify insurance coverage, benefits, and hospice eligibility; collaborate with billing teams as needed.
• Work closely with nurses, social workers, and external providers to facilitate timely admissions.
• Ensure all intake processes meet Medicare, Medicaid, and state regulatory requirements.
• Maintain consistent communication with referral sources and families to provide updates and ensure continuity of care.
• Complete and submit all intake and admission paperwork in a timely and accurate manner.
