About The Position

James River Hospice is seeking a Full-time Hospice Intake Coordinator to join their team. This role manages the referral and admission process for patients entering hospice services. The position involves working closely with patients, families, referral sources, clinical staff, and leadership to ensure a smooth and timely transition into hospice care, while providing exceptional customer service and support. This is a full-time, in-office position with a set schedule and participation in a rotating weekend and holiday schedule.

Requirements

Manages the referral and admission process for patients entering hospice services.
Works closely with patients, families, referral sources, clinical staff, and leadership.
Ensures a smooth, timely transition into hospice care.
Delivers exceptional customer service and support.
Verifies patient eligibility and insurance coverage.
Ensures all required documentation is completed for timely admission.
Completes daily follow-up on pending referrals.
Provides admission status updates to leadership.
