About The Position

James River Hospice is seeking a Full-time Hospice Intake Coordinator to join their team. This role manages the referral and admission process for patients entering hospice services. The Hospice Intake Coordinator works closely with patients, families, referral sources, clinical staff, and leadership to ensure a smooth, timely transition into hospice care while delivering exceptional customer service and support.

Requirements

• Availability for a rotating weekend and holiday schedule

Responsibilities

• Coordinates hospice referrals and admissions, verifies patient eligibility and insurance coverage, communicates with patients, families, and referral sources, and ensures all required documentation is completed for timely admission.
• Completes daily follow-up on pending referrals and provides admission status updates to leadership.
• Other duties as assigned.
