About The Position

Join Agrace, a nationally recognized nonprofit leader in hospice and palliative care. This is an opportunity for a CNA who values meaningful, patient-centered care to make a difference. The CNA IPU is responsible for patient and family satisfaction by assisting with activities of daily living, providing light homemaker help, following the plan of care, communicating changes in condition, and documenting care. The CNA IPU provides patient and family centered care consistent with Agrace philosophy, goals, standards, regulations and policies.

Requirements

  • High school diploma or GED, or equivalent combination of education and work experience
  • Graduation from an accredited Certified Nursing Assistant program
  • Current, unrestricted Nursing Assistant Certification in state of employment
  • Must be at least 18 years of age
  • Basic Microsoft Office skills
  • Experience using electronic health records
  • Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property
  • Ability to use proper body mechanics, safe patient handling, and standard precautions

Nice To Haves

  • One-year personal care experience, hospice, or home health

Responsibilities

  • Assist Patients with Activities of Daily Living (ambulation, changing position, moving to/from sitting and standing, bathroom functions, bathing, showering, dressing, oral care, grooming, eating and drinking, post mortem care, obtaining vital signs, assisting with treatments, anticipating and responding to patient needs, rounding on patients).
  • Provide homemaker assistance (changing linens, doing laundry, tidying room, removing trash, delivering patient supplies, delivering food and beverages).
  • Communicate by reading, understanding, and following the plan of care, documenting cares and changes in patient condition in electronic health record, documenting and reporting significant changes in condition or unusual incidents to appropriate Interdisciplinary Team Member, and reinforcing patient and family education.
  • Participate in meetings to plan and evaluate patient/family services.
  • Precept new staff and students.
  • Delegate tasks to volunteers as appropriate.
