About The Position

The Hospice IPU RN is responsible for facilitating the transition of patients from hospital care to hospice services, ensuring that they receive comprehensive and compassionate end of life care. This role involves coordinating with healthcare providers, patients and families to provide a seamless transition and support throughout the hospice care process. This position will work on-site at Emory St. Josephs hospital, working 7:00pm -7:00am; 7 days on / 7 days off. This position will manage and oversee the care plan and provide evening rounds if needed and urgent on call needs. Requirements

  • Nursing degree from an accredited nursing program
  • Current active Georgia nursing license as a registered nurse
  • Minimum of 3 years of experience in an acute hospital setting
  • Strong interpersonal and communication skills, with the ability to handle sensitive situations.
  • Managing situations that require compassion and empathy.
  • Ability to work collaboratively with healthcare teams and families
  • Must be able to manage multiple tasks and priorities effectively.
  • Proficiency in electronic health records (EHR/EMR).
  • Acceptance of and adaptability to different social, racial, cultural and religious modes.
  • Must be able to work independently, make judgements based on assessments and plan accordingly.
  • Must be flexible and be able to cope with mental and emotional stress and demonstrate emotional stability.
  • Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and possibly hazardous materials.

Nice To Haves

  • 1-2 years of hospice experience and/or IPU experience a plus.

Responsibilities

  • Manage the transition process for patients moving from hospital care to hospice services, ensuring that all aspects of care are addressed.
  • Coordinate with hospice and hospital teams to develop and implement personalized care plans for patients.
  • Manage patients transition after passing to include pronouncement, coordination of the funeral home and assisting family during their period of grief.
  • Serve as the primary point of contact between the hospital, hospice team, patients and their families.
  • Facilitate clear and compassionate communication regarding patient care plans, goals, and hospice services.
  • Provide support to hospital and hospice staff on best practices for managing end-of-life care and transitions.
  • Offer emotional and informational support to patients and families throughout the hospice care process.
  • Ensure that the transition process complies with regulatory requirements and hospice care standards.
  • Monitor patient outcomes and satisfaction to identify areas for improvement in the transition process.
  • Participate in quality improvement initiatives and contribute to developing best practices in hospice care.
  • Maintain accurate and confidential documentation related to patient transitions and hospice care coordination.
  • Stay informed about updates in hospice care regulation and best practices.
