About The Position

Croí Health is seeking a compassionate, collaborative, and experienced Hospice House Chaplain to join the interdisciplinary team at the Pat Roach Hospice House. This role provides spiritual care in a home-like hospice setting, supporting residents and their loved ones through life's sacred moments. The chaplain also offers encouragement and support to the staff and volunteers who provide compassionate care.

Requirements

Minimum of two (2) units of Clinical Pastoral Education (CPE) required.
Strong communication, listening, and interpersonal skills.
Knowledge of the spiritual, emotional, and psychosocial issues related to serious illness, dying, death, and bereavement.
Ability to work collaboratively, maintain professional boundaries, and thrive as part of an interdisciplinary team.

Nice To Haves

Board Certification preferred.
Hospice or healthcare chaplaincy experience preferred.

Responsibilities

Provide compassionate spiritual care and emotional support to residents and their loved ones throughout the end-of-life journey.
Conduct spiritual assessments, counseling, and supportive visits while honoring the diverse beliefs, cultures, values, and traditions of those we serve.
Be a calming and compassionate presence for families navigating anticipatory grief, dying, and loss.
Collaborate with the Hospice House team and the broader interdisciplinary team to provide whole-person, patient-centered care.
Offer encouragement and support to the staff and volunteers whose daily work is centered on caring for others.
Participate in interdisciplinary team meetings, case conferences, and quality improvement activities.
Complete timely clinical documentation.
