Requirements

  • High School Diploma, or GED equivalent.
  • Current Home Health Aide certification.
  • Valid CPR Certification.
  • Certified to pass Home Health Aide Competency Test.
  • Excellent verbal and written communication skills.
  • Excellent interpersonal and customer service skills.
  • Excellent organizational skills and attention to detail.
  • Excellent time management skills, with a proven ability to meet deadlines.
  • Strong analytical and problem-solving skills.
  • Ability to prioritize tasks, and to delegate them when appropriate.
  • Ability to function well in a high-paced, and at times, stressful environment.
  • Proficient with Microsoft Office Suite or related software.

Nice To Haves

  • 1 year of Home Health Aide or LTCF experience preferred.

Responsibilities

  • Provides patient care, ensuring patient safety and comfort, according to the Home Health Aide assignment form.
  • Reports and records any changes in condition or behavior of the patient.
  • Adapts to changes in workload or patient assignments without interference to patient care.
  • Responsible for completion of assignments, maintaining quality work.
  • Establishes good rapport with patients and families, respecting patients rights to privacy and confidentiality of information.
  • Accurately communicates in writing through utilization of appropriate charting methods, and turns in all paperwork on time.
  • Provides support to patients and families in a courteous manner.
  • Performs routine duties without physical supervision.
  • Accurately communicates patients information to the RN Case Manager, LPN and/or Assistant Director of Clinical Services.
  • Assists in maintaining equipment assigned to patient care, and reports malfunctioning equipment to appropriate personnel.
  • Any and all other duties and responsibilities as assigned.
