About The Position

This is a full-time, 1st shift position for a Hospice Home Health Aide. The role involves providing competent patient care according to the RN's plan of care, reporting changes in patient condition, and understanding the hospice philosophy. Responsibilities include assisting with personal care needs, meeting safety needs, taking vital signs, providing skin care, and changing linens. The position requires effective communication and documentation skills, adherence to patient confidentiality (HIPAA/Privacy acts), professionalism, and flexibility in scheduling. The aide will participate in quality improvement processes and ensure patient satisfaction. Adherence to regulatory requirements, including annual in-service hours, is mandatory.

Requirements

  • Must be certified as a nurse assistant in the state in which the teammate practices
  • Nurse assistant in good standing with the State Nurse Aide Registry with eligibility to work in home health agencies and hospices
  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months
  • A valid driver’s license issued by the Division of Motor Vehicles.
  • Complete all training for Advocate Aurora Hospice, as well as maintain all annual training per individual state requirements.

Responsibilities

  • Provide competent patient care assigned by RN and plan of care.
  • Report all changes in patient condition to RN or manager.
  • Understand the hospice philosophy and be comfortable providing specialized care to the terminally ill.
  • Perform personal care needs including feeding patient, laundry, light cleaning of patient room and washing.
  • Meet safety needs of patient using equipment safely and properly.
  • Take temperatures, pulse, respiration and blood pressure as directed by the RN when assigned.
  • Provide skin care and change patient position to prevent breakdown as assigned by RN.
  • Change patient bed linens and clothing as requested by the patient and family.
  • Demonstrate effective communication skills, verbally and in written documentation, per agency, state, federal and accrediting organizations, within the assigned time frames.
  • Utilize appropriate communication skills and effective interpersonal skills with all customers.
  • Prepare documentation reflecting patient’s care; adhere to documentation as required by agency, state, federal, and Accrediting Organization standards within required time frames.
  • Maintain patient confidentiality, observing of HIPAA/ Privacy acts.
  • Demonstrate professionalism and work in a cooperative manner with all associates.
  • Demonstrate flexibility with scheduling, commitment to your patient, team, and organization.
  • Demonstrate flexibility in scheduling to support department and other service offices when needed.
  • Demonstrate self-direction for maintaining home health aide skills.
  • Demonstrate commitment and support team, office, company and organizational goals.
  • Participate actively in CQI process to improve processes within the organization, upon request.
  • Assist with problem identification and solutions.
  • Participate in quality, improvement projects when assigned.
  • Demonstrate measures to ensure patient satisfaction.
  • Adhere to regulatory requirements.
  • Maintain annual in-service hours, per state/federal regulations, Hospice education hours and OSHA requirements.
  • Provide hospice home health aide services in accordance with all regulatory rules and standards.
