About The Position

This role is based out of the Onslow and Pender office locations. These offices serve Pender, Duplin, Onslow, Jones, and Carteret counties. Orientation will include some on-site training at the Corporate office location in Wilmington, NC. Requirements

Graduate of an approved school of nursing, either through an AD, Diploma or BSN program.
Current license to practice professional nursing in the State of North Carolina / South Carolina.
Two years of experience.

Home Care or Hospice or related oncology experience.

Responsibilities

See patients wherever they consider home (personal residence, assisted living facilities, skilled nursing facilities).
Provide quality care regardless of circumstance to a variety of different socioeconomic and cultural backgrounds.
Train caregivers on how to care for their loved one, including demonstrating wound care, medication administration, personal care, and end-of-life expectations.
Spend extended time periods driving between patients, documenting, and coordinating care.
Coordinate care between patient's community medical provider, hospice provider, and other disciplines to provide updates and obtain new orders.
Receive patient assignments based on team needs, determined by acuity, geography, and other factors.
Perform real-time documentation in patient's home.
Order medications and supplies online.
Self-schedule based on patient's needs and acuity, with flexibility to assist the team as needed.
Participate in an Interdisciplinary Team (IDT) meetings every two weeks to discuss patient eligibility.
