About The Position

The Float Hospice Registered Nurse (RN) provides skilled nursing care and support to hospice patients and families across multiple service areas, care teams, and locations as needed. This role serves as a flexible clinical resource, filling staffing gaps, supporting admissions, routine visits, continuous care, while ensuring compassionate, patient-centered end-of-life care consistent with hospice philosophy and regulatory requirements. The Float RN collaborates with the interdisciplinary team to assess, plan, implement, and evaluate individualized care plans focused on comfort, symptom management, dignity, and quality of life for terminally ill patients.

Requirements

  • Current, unrestricted RN license in the State of Louisiana (or applicable state of practice).
  • Graduate of an accredited nursing program.
  • Minimum 1–2 years of RN clinical experience required.
  • Experience in hospice, home health, oncology, geriatrics, med-surg, or palliative care.
  • EMR systems and mobile charting.
  • Understands Hospice Philosophy.
  • Must have experience with issues concerning terminally ill and death/dying.
  • Valid driver’s license, reliable transportation, and proof of auto insurance.
  • Current CPR certification.
  • Ability to travel throughout assigned service areas

  • Hospice and Palliative Credentialing Center certification (CHPN) preferred

Responsibilities

  • Provide direct nursing care to hospice patients in homes, facilities, assisted living communities, nursing homes, or inpatient hospice settings.
  • Float between assigned geographic territories, teams, and patient caseloads based on organizational needs.
  • Conduct comprehensive patient assessments, including physical, emotional, psychosocial, and spiritual needs.
  • Develop, implement, and update individualized plans of care in collaboration with the interdisciplinary hospice team.
  • Perform skilled nursing interventions including: Pain and symptom management, Medication administration and education, Wound care, Catheter and ostomy care, Disease progression monitoring.
  • Support hospice admissions, recertifications, and routine nursing visits.
  • Educate patients, caregivers, and families regarding disease progression, medications, comfort care, and end-of-life expectations.
  • Coordinate care with physicians, nurse practitioners, social workers, chaplains, CNAs, and other healthcare providers.
  • Respond to urgent patient needs and provide crisis intervention as necessary.
  • Complete accurate, timely clinical documentation in the electronic medical record (EMR).
  • Participate in on-call rotation, weekend coverage, and holiday scheduling as assigned.
  • Ensure compliance with hospice regulations, Medicare Conditions of Participation, state nursing regulations, and agency policies.
  • Promote patient dignity, comfort, and family support throughout the hospice journey.
