About The Position

Fortis Health is a clinician-led organization that supports home health and hospice agencies across multiple states, including Valeo Home Health in Utah. The Hospice Director of Nursing (DON) leads and supports clinical nursing services across the hospice agency, overseeing clinical operations and ensuring high-quality, patient-centered care. The role provides leadership, guidance, and professional development to clinical staff while monitoring performance, ensuring compliance, and supporting accountability. The position also oversees patient care processes, quality outcomes, patient safety, infection control, and coordination across disciplines. It participates in agency planning, growth initiatives, and community outreach. Responsibilities and qualifications may evolve with the organization or regulatory requirements.

Requirements

Licensed Registered Nurse (RN) in the state of practice.
Bachelor’s degree in Nursing or a related field.
At least 1 year of management or supervisory experience in a clinical setting.
At least 2 years of Director of Nursing experience in hospice or home health.
At least 5 years of hospice clinical experience.
