Hospice Coordinator
About The Position
A facility near La Crosse, WI is seeking a Hospice Coordinator to join their hospice and palliative care team! This is a full-time, permanent position with a day shift schedule, Monday through Friday. A signing bonus is available. The Hospice Coordinator oversees all aspects of hospice patient care, collaborating with the interdisciplinary team to develop, implement, evaluate, and adjust individualized care plans while ensuring quality symptom management, regulatory compliance, and continuity of care across settings. This role also provides clinical leadership and supervision for hospice staff and volunteers, including staffing, training, performance management, quality assurance, admissions coordination, patient/family education, and maintaining positive relationships with patients, families, physicians, facilities, and community partners.
Requirements
- RN license with the privilege to practice in the state of Wisconsin
- 2 or more years of clinical nursing experience required
Nice To Haves
- Experience in Hospice preferred
- Experience in management preferred
