Responsibilities

Coordinate care plans for hospice patients.
Collaborate with interdisciplinary teams.
Ensure the highest quality of care for hospice patients.

Work closely with Hospice and Long-term care facilities to ensure that patients receive the necessary medical equipment for their unique needs.
Serve as a liaison between facilities and insurance providers, utilizing strong organizational and communication skills to verify coverage and navigate the insurance process.
Provide exceptional service to patients and their families, ensuring they have access to vital equipment necessary for their comfort and care.
