About The Position

Halcyon Home is seeking a dedicated, compassionate Hospice CNA to support its geriatric hospice population in Tomball, Texas, and surrounding areas. The CNA provides direct patient care under the direction of an RN and in accordance with the Aide Plan of Care. The role involves supporting patients with transfers, ambulation, nutrition, fluids, treatments, and procedures while helping maintain a clean, safe, and comfortable environment. The CNA also documents care, reports changes, follows infection-control practices, and communicates respectfully with patients, families, and colleagues. The position is full-time and pays $24–$26 per hour.

Requirements

Current CNA certification in Texas.
Strong interpersonal and communication skills.
A kind and patient demeanor with a commitment to providing dignified care.

Nice To Haves

Proven experience in hospice or senior care is preferred.
Hospice experience, or experience in a related field with a desire to learn about home healthcare.

Responsibilities

Provide direct patient care under the direction of the RN and according to the Aide Plan of Care.
Assist patients with safe transfers and ambulation in accordance with Hospice policy.
Report patient changes and document pertinent information and care provided to support continuity of care.
Practice accepted infection-control principles.
Provide a clean, safe, and comfortable environment.
Communicate positively, supportively, and respectfully with patients, families, and other employees.
Respect patients’ privacy and property.
Assist patients with proper nutrition and adequate fluid intake.
Contribute to efficient Hospice operations and demonstrate effective time-management skills.
Perform treatments and procedures according to Hospice policy.
Promote the Hospice philosophy and administrative policies to support quality of care.
