About The Position

St. Anthony’s is seeking compassionate and dedicated Hospice Care Aides to join our home hospice team in providing exceptional end-of-life care throughout our community. This role requires a balance of clinical support skills, emotional intelligence and compassion to ensure patients receive comfort, dignity and support during one of life’s most difficult journeys. Position Summary: The Hospice CNA / SRNA provides compassionate personal care and supportive services to hospice patients in their homes, assisted living facilities, nursing facilities and other residential care settings under the direction and supervision of the licensed nursing team. Care is provided in accordance with each patient’s individualized Plan of Care, organizational standards and hospice regulations. The Hospice CNA / SRNA plays an important role in supporting patient comfort, dignity and quality of life while assisting patients and families throughout the hospice journey.

Requirements

  • Must possess an active Kentucky State Registered Nurse Aide (SRNA) certification.
  • Demonstrated commitment to compassionate, patient-centered care.
  • Strong interpersonal and communication skills.
  • Ability to work independently while collaborating effectively within an interdisciplinary team.
  • Basic computer proficiency including EMR documentation required.
  • Valid driver’s license, reliable transportation and proof of automobile insurance required.
  • Must meet all legal and regulatory requirements of both federal and state governments as well as Medicare Hospice Conditions of Participation.
  • Must be able to frequently move throughout patient homes and residential care environments to provide patient care.
  • Must be able to communicate verbally, electronically and in writing with patients, families and team members.
  • Must be able to lift, transfer, push and pull up to 50 pounds as necessary to assist with patient care activities.
  • Must be able to safely enter and navigate varying home environments.
  • Regularly exposed to blood, body fluids, cleaning chemicals and other potentially hazardous substances while providing patient care.
  • Must be able to wear appropriate Personal Protective Equipment.
  • Must be able to operate medical equipment, computers and standard office equipment.

Nice To Haves

  • Previous direct patient care experience in a healthcare setting preferred.
  • Hospice, home health, long-term care, rehabilitation or geriatric care experience preferred.

Responsibilities

  • Provides personal care services including bathing, grooming, dressing, toileting, oral hygiene, feeding assistance and mobility support.
  • Assists patients with repositioning, transfers, ambulation and range-of-motion activities to support comfort and safety.
  • Observes and reports changes in patient condition, behavior or comfort level to the licensed nursing team.
  • Assists with obtaining vital signs and other patient care tasks as directed by nursing staff.
  • Provides care in patient homes and residential care environments while maintaining a safe, clean and supportive care setting.
  • Provides compassionate emotional support and respectful interactions with patients and families.
  • Assists in maintaining patient dignity, privacy and comfort throughout all aspects of care.
  • Documents care provided and patient observations accurately and timely within the electronic medical record system.
  • Collaborates with assigned nurses and interdisciplinary team members to support individualized patient care plans.
  • Travels throughout the service area to provide patient care as assigned.
  • Participates in staff meetings, trainings and educational opportunities as assigned.
  • Maintains confidentiality and complies with all organizational, state, federal and Medicare regulatory requirements.
  • Performs additional duties as assigned.
