About The Position

The Clinical Navigator at Lifted Hospice plays a crucial role in guiding patients, families, and providers through the initial steps of hospice care with clarity, compassion, and confidence. This position combines nursing expertise with strong communication and outreach skills to assess eligibility, provide education, coordinate admissions, and ensure smooth transitions into hospice care. The Clinical Navigator is often the first clinical contact, offering reassurance and peace during a potentially overwhelming time. They also act as a liaison between patients, families, referral partners, and the interdisciplinary team, ensuring clear communication throughout the admission process. A key responsibility includes facilitating and managing General Inpatient (GIP) admissions, ensuring patients receive the appropriate level of care while adhering to clinical excellence and regulatory compliance. This role is vital in ensuring more people receive timely and appropriate hospice care.

Requirements

• Current, unrestricted RN license in the State of Texas, in accordance with Nurse Licensure Compact rules.
• Minimum of 2 years of clinical nursing experience required.
• Experience collaborating with physicians, hospitals, and interdisciplinary healthcare teams.
• Strong understanding of hospice philosophy, eligibility criteria, Medicare regulations, and Conditions of Participation (CoPs).
• Working knowledge of CHAP standards and Texas hospice regulations.
• Knowledge of General Inpatient (GIP) documentation requirements and level-of-care criteria.
• Excellent clinical assessment, critical thinking, and decision-making skills.
• Ability to navigate emotionally sensitive conversations with empathy, professionalism, and confidence.
• Strong communication, organizational, and time-management skills.
• Proficiency with electronic medical records (EMR) and Microsoft Office applications.
• Ability to work independently while collaborating effectively with interdisciplinary teams.
• Must be able to read, write, and communicate clearly in English.
• Current driver’s license, reliable transportation, and valid auto liability insurance.

Nice To Haves

• Bachelor of Science in Nursing (BSN) preferred.
• Experience with hospice admissions and General Inpatient (GIP) level-of-care management preferred.

Responsibilities

• Provide hospice education to patients, families, and caregivers, guiding them through services, levels of care, and expectations.
• Conduct clinical assessments to evaluate patients for hospice eligibility and facilitate admissions.
• Coordinate admissions by collaborating with physicians, the Medical Director, and the interdisciplinary team.
• Complete accurate and timely documentation to support hospice eligibility and regulatory requirements.
• Assess patients for General Inpatient (GIP) eligibility, coordinate admissions, conduct daily GIP visits, and ensure documentation supports the appropriate level of care.
• Develop relationships with referral partners (hospitals, physicians, case managers, discharge planners) by providing education about Lifted Hospice services.
• Partner with Hospice Liaisons to strengthen referral relationships through clinical education and community engagement.
• Uphold Medicare, CHAP, HIPAA, and Texas regulatory standards.
• Maintain accurate EMR documentation and deliver compassionate, patient-centered care.
