A Calling with Impact At Lifted Hospice, our Clinical Navigators help patients, families, and providers take the first step into hospice care—with clarity, compassion, and confidence. As a Clinical Navigator, you’ll combine your nursing expertise with strong communication and outreach skills to assess eligibility, provide education, coordinate admissions, and facilitate seamless transitions into hospice care. You are often the first clinical face patients see—bringing reassurance, presence, and peace to what can be an overwhelming moment. You’ll also serve as a trusted clinical liaison between patients, families, referral partners, and the interdisciplinary team, ensuring communication remains clear throughout the admission process. In addition, you’ll play an essential role in facilitating and managing General Inpatient (GIP) admissions, helping patients receive the appropriate level of care while maintaining the highest standards of clinical excellence and regulatory compliance. Your presence and leadership help ensure more people receive the care they need—at the right time, in the right way. Your Role in the Mission As a Clinical Navigator, you combine clinical expertise with compassionate guidance to help patients and families navigate the transition into hospice care. You assess eligibility, provide education, coordinate admissions, and collaborate with physicians, the Medical Director, Hospice Liaisons, and the interdisciplinary team to ensure timely, seamless care. You also play a key role in General Inpatient (GIP) care by evaluating eligibility, coordinating admissions, conducting daily GIP visits, and maintaining accurate, compliant documentation. Through education, relationship-building, and clinical excellence, you strengthen referral partnerships while representing Lifted Hospice with compassion, integrity, excellence, innovation, and joy.
