About The Position

Touchstone Hospice, part of the CareM network, is seeking a dynamic, experienced RN to lead our clinical team in San Antonio. As our Hospice Clinical Manager, you’ll drive quality, compassion, and coordination across every patient experience — guiding a mission-driven team that shows up for patients and each other, every single day.

Requirements

Licensed RN with a degree from an accredited school of nursing
Minimum 1 year of hospice experience
Minimum 1+ year of clinical leadership or supervisory experience
Proven ability to coach teams, manage care delivery, and support emotionally complex situations
Strong judgment, problem-solving skills, and regulatory understanding
Compassionate, decisive, and unafraid to lead from the front
Able to manage documentation, compliance, and care coordination with confidence
Willing to share on-call responsibilities as needed
Commitment to excellence, empathy, and collaboration

Responsibilities

Supervise clinical operations and ensure high-quality hospice care is delivered in accordance with physician orders and patient needs
Mentor and lead nursing and support staff across interdisciplinary care teams
Oversee patient plans of care and ensure documentation compliance with agency policy and regulatory standards
Participate in hiring, onboarding, and performance evaluations for clinical team members
Collaborate with physicians, referral partners, and families to ensure exceptional communication and service
Serve as a resource and escalation point for clinical challenges and quality concerns
Coordinate patient assignments, manage staffing needs, and participate in on-call rotation
Step in for the Director of Clinical Services as needed
