Requirements

  • Graduate of an accredited nursing school.
  • Active RN license in the state of practice.
  • 2+ years RN experience (Hospice preferred).
  • 2+ years in a supervisory role.
  • Current CPR/BLS certification.
  • Valid driver’s license and insured personal vehicle.
  • Proficiency with Microsoft Office.
  • Knowledge of state and federal home health regulations.
  • Ability to read, follow, and document clinical instructions in an EMR.
  • Strong leadership, communication, and coaching skills.
  • Ability to work professionally with adult patients.
  • Flexibility and strong problem‑solving abilities.
  • Physical and mental stamina; required health clearances (including TB/PPD or chest X‑ray).

Responsibilities

  • Oversee daily clinical operations and ensure high‑quality patient care.
  • Participate in patient eligibility decisions with physicians and referral partners.
  • Ensure thorough assessments and appropriate clinical care plans at admission.
  • Lead case conferences and support strong communication across the care team.
  • Review electronic clinical records for quality and compliance.
  • Coordinate staffing to meet patient needs and optimize clinician productivity.
  • Assign patients by clinician territory and reduce service overlap.
  • Partner with HR to anticipate staffing needs and recruit clinicians.
  • Track and analyze clinician productivity data.
  • Implement new clinical and regulatory guidelines with Quality Assurance.
  • Document care accurately in the EMR.
  • Participate in ongoing training and education.
  • Adhere to infection‑control, HIPAA, and professional standards.
  • Participate in the Hospice Administrator On‑Call rotation.
  • Other duties as assigned.
