Requirements

  • Licensed / Registered Professional in the state of California.
  • Associate degree or higher from an accredited institution.
  • Recent clinical experience within Hospice.
  • Working knowledge of HCHB (EMR).
  • Demonstrated ability to work effectively in a team.
  • Excellent observation, verbal, and written communication skills.
  • Knowledge of State and Federal regulations in Hospice.

Responsibilities

  • Maintaining and implementing education for all clinical staff.
  • Working closely with Branch Directors and Field Staff Supervisors (FSS) to monitor educational standards.
  • Monitoring documentation and clinical skills.
  • Setting up workshops for ongoing education.
  • Working with FSS on clinical orientation processes.
  • Facilitating consistency of standardized education policies, procedures, and programs.
  • Partnering with Clinical Operations, Performance Improvement, Compliance, and Service Operations to ensure education and training align with current clinical standards, guidelines, statutes, rules, and regulations.
  • Developing clinical competencies.
  • Developing, implementing, and evaluating the orientation program for new personnel, and training programs to expand staff capacity through in-services and continuing education.
  • Focusing on PDGM (Patient Driven Groupings Model) and HVVBP education.
  • Implementing practices identified to improve clinical delivery, quality, and service.
  • Assessing learning needs and competence of staff to improve performance, teaching skills, and quality of care, and assuring compliance with accreditation standards.
  • Developing curriculum and ability to precept.
  • Trending and analyzing utilization, outcomes, and quality of program data on an ongoing basis.
  • Working closely with Branch Directors, Rehab Directors, and Quality Care Managers to monitor educational standards and deficits.
  • Reviewing clinical documentation and skills for ongoing learning needs.
  • Ensuring new hire education and ongoing staff training align with current clinical standards, guidelines, statutes, rules, and regulations.
  • Providing education and support to clinical staff in maintaining quality improvement outcomes and STAR ratings.
  • Participating in the development, implementation, and evaluation of best practices for Home Care/Hospice.
  • Assisting Clinical Managers in evaluating the performance of new clinicians.
  • Regularly evaluating the appropriateness of educational courses assigned to staff.
  • Facilitating learning experiences for staff through direct and indirect methods.
  • Ability to work and communicate via electronic communications.
  • Possess knowledge of HCHB, SHP, Microsoft Word, Excel, and PowerPoint.
