Requirements

  • Graduate of an accredited seminary or school of theology (BD, MDiv, Master of Theology or equivalent theological degree).
  • Excellent interpersonal and communication skills.
  • Ability to work as a member of an interdisciplinary group.
  • Ability to work with socio-culturally diverse patients/families of all faiths and accept different lifestyles, cultures, beliefs, and values.
  • Ability to network with community clergy and congregations.
  • Knowledge of and commitment to hospice philosophy of care.
  • Proficiency in electronic documentation.
  • Must be willing to operate one’s own vehicle to drive within the patient service area.

Nice To Haves

  • Four units of Clinical Pastoral Education preferred.
  • Experience working with death and dying, individuals/family/caregivers preferred.

Responsibilities

  • Performs spiritual assessment on all Hospice patients and families who desire spiritual support based on their religious, spiritual and cultural practices and beliefs.
  • Acts as liaison to and resource for community clergy; coordinates with community clergy to arrange participation in spiritual rituals and pastoral support.
  • Provides nondenominational spiritual counseling per patient and family, providing support through discussion of spiritual and existential issues which may include prayer, counsel and support.
  • Responds to the spiritual, social and emotional needs of terminally ill patients and their families as it relates to their spiritual/existential needs.
  • Functions effectively as part of the interdisciplinary team, attending and participating in staff and interdisciplinary team meetings.
  • Documents in EMR and in writing of services provided to patient and family in a complete and timely manner.
  • Provides bereavement follow-up care as needed in conjunction with bereavement coordinators.
  • May assist with bereavement support groups and provides referrals to family for community grief support groups or individual counseling as needed.
  • Actively participates in Quality Assessment and Performance Improvement (QAPI) activities.
  • Represents the organization in a positive and professional manner in the community, communicating the mission, ethics and goals of our organization.
