About The Position

Are you an experienced healthcare Chaplain seeking an opportunity to support individuals in a meaningful way? Join us! Walla Walla Community Hospice is seeking a Spiritual Support Counselor (Hospice Chaplain) to join our interdisciplinary care team. The Spiritual Support Counselor (SSC) provides spiritual assessment, counseling, and support to patients and families in accordance with the patient’s beliefs and preferences. This position collaborates closely with nursing, social work, and medical providers to ensure holistic end-of-life care. The ideal candidate demonstrates emotional intelligence, cultural humility, and the ability to support individuals of all faith traditions or no faith tradition. Purpose The Spiritual Support Counselor (SSC) is responsible for providing spiritual and emotional support and counseling to patients, families, and care teams throughout the end-old-life journey. The SSC facilitates connection with local clergy, pastoral counselors, or other individuals who can support the patient’s spiritual needs. The SSC also serves as an integral member of the interdisciplinary team.

Requirements

Current knowledge of the principles and practices of spiritual support and professional chaplaincy.
Exhibits excellent written and verbal communication skills, Always exhibit compassion, cultural awareness, empathy, integrity and ethics, emotional strength, organizational skills, problem solving, and critical thinking.
Must function as a strong patient advocate in support of their spiritual wellness during hospice care.
Ability to maintain confidentiality of information related to the WWCH and its employees.
Maintain familiarity with WWCH policies and procedures, as well as Federal, State, and local regulations that govern hospice agencies.
Must have a valid driver’s license, auto insurance, a safe driving record, the use of a reliable vehicle, and the ability to drive year-round within our services area which includes Walla Walla County, Columbia County, and northeastern Umatilla County.
Must exhibit effective time management and ability to work autonomously with accountability for excellence in work product.
Strong decision-making and critical thinking skills.
Ability to connect well with others.
Demonstrates interfaith spiritual confidence and humility.
Working knowledge of hospice philosophy, benefits, and services.
Proficiency in reading, writing, comprehending, and speaking English as required for business necessity.
Proficiency with MS O365, computer skills, and familiarity with electronic health records.
Proficient in medical terminology
Accountability for HIPAA privacy and security standards within the organization
Bachelor’s degree (B.A.) in Theology or related field of study.
Master’s degree in applied theology, Divinity, Pastoral Care or related field of study, OR two years related experience and/or training; or equivalent combination of education and experience in religious studies, theology, philosophy, ministry, counseling, or professional chaplaincy.
Valid State Driver’s license
CPR certification

Responsibilities

Assess the Hospice patient and support system to identify spiritual needs as evidenced by the Plan of Care (POC) documentation and the IDT/IDG conference.
Provide direct patient spiritual support in accordance with applicable standards of care, and as defined by the Hospice model of care.
Assess, plan, and facilitate open and honest discussions with patients, those involved in their care, and fellow Interdisciplinary Team members, about the death and the dying process.
Coordinate with patients, family members, and others involved in the patient’s care to provide support navigating complex emotions.
Initiate, complete, and routinely update the patient plan of care in accordance with Hospice Chaplain best practices.
Continuously plan, implement, and evaluate patient needs, incorporating the patient, along with those involved patient’s care, in development and updates to the Plan of Care.
Initiate and maintain effective communication with the Interdisciplinary Team, WWCH staff members, and outside stakeholders (such as community clergy) to coordinate optimal spiritual support and use of resources for patients and those involved in their care.
Participate in WWCH orientation and training programs for professional staff.
Engage professionally with SSC colleagues and interdisciplinary team members in all team or agency staff meetings.
Server as SSC preceptor for new team members, as needed and requested by Leadership.
Participate in agency and community programs as requested to promote WWCH within our community and improve community understanding of hospice care.
Performs other duties as assigned.
