About The Position

This role plans and coordinates spiritual services for Hospice families and patients. The Hospice Chaplain and Volunteer Coordinator provides spiritual support based on identified needs, assesses spiritual concerns, and ensures spiritual support aligns with patient wishes. The position also involves providing consultation and education to hospice team members, healthcare facilities, and the community. Additionally, the role performs sacerdotal functions to address spiritual crises, assists with volunteer training, serves on the Interdisciplinary Group to develop care plans, provides on-call chaplaincy coverage, acts as a liaison to community clergy, and offers grief and bereavement counseling. The position requires adherence to company policies, state and federal laws, and maintaining confidentiality. The role also involves coordinating patient care responsibilities and performing other duties as assigned.

Has never been shown by credible evidence (e.g., a court and jury, a department investigation, or other reliable evidence) to have abused, neglected, sexually assaulted, exploited, or deprived any person or to have subjected to any person to serious injury as a result of intentional or grossly negligent misconduct as evidenced by an oral or written statement to this effect obtained at the time of application.
Ability to work effectively independently and within an interdisciplinary team.
Orientation to United Hospice and any other training that may be necessary to complete the assigned duties effectively.
Participate in center/agency surveys (Licensure / JCAHO) and any subsequently required reports.
Attend and participate in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification, as required.
Attend and participate in mandatory inservices.
Honor patients/residents’ rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints.
Comply with corporate compliance program.
Report job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary.
Follow established safety regulations, to include fire protection & prevention, smoking regulations, infection control, etc.
Follow established safety procedures when performing tasks and/or working with equipment.
Perform other related duties as necessary and as directed by supervisor.
Completion of a Master’s degree from an accredited theological school/seminary.
Completion of 1-4 units in Clinical Pastoral Education from a nationally recognized provider.
Two years of ministerial experience.
Appropriate endorsement within his/her denomination (e.g., ordination).
Previous training and/or experience in death and loss issues.
Current Driver’s License and Automobile Liability Insurance

Responsibilities

Provide spiritual services to patients and families based upon their identified need (spiritual values and beliefs are NOT to be imposed upon the patient/family).
Assess the spiritual needs/concerns of patients and families through direct communication with the individual, the patient’s clergy, or by information obtained by other Hospice personnel.
Ensure that spiritual support is available to patients and families in keeping with their wishes and desires.
Provide consultation education and support for other members of the hospice team, healthcare centers and facilities, and the community.
Performs sacerdotal functions as necessary to palliate the spiritual crises of patients and families.
Assist with volunteer training as requested.
Serve as a member of the Interdisciplinary Group to assist in identifying and addressing spiritual needs and developing care plans, including attendance at team conferences for residents of healthcare centers and nursing facilities.
Assist with on-call chaplaincy coverage.
Act as a liaison to community clergy.
Provide grief and bereavement counseling to families.
Conduct memorial/funeral services as requested.
Keep supervisor informed of daily activities and issues.
Must get along with coworkers, maintain effective relationships with management staff, employees, and the general public.
