About The Position

Residential Home Health and Hospice is a nationally recognized leader in home health and hospice, and a consistent Top Workplace. We are hiring a Certified Nursing Assistant for a Hospice position providing care to patients in their homes in the Milford, PA area. This position is eligible for a $4,000 sign-on bonus. The role involves delivering compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life. Responsibilities include providing hands-on care, assisting with activities of daily living (ADLs), observing and reporting changes in condition, supporting and guiding patients and caregivers, responding promptly to patient needs, and accurately documenting care provided in the EMR. The role requires travel to patient homes within an assigned territory.

Requirements

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
• Legally authorized to work in the United States

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life
• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• Travel to patient homes within assigned territory
