About The Position

Heritage Hospice is hiring a PRN Hospice CNA to join its interdisciplinary care team as it resumes services in the Albuquerque community. The CNA provides personal care, comfort, and supportive services to hospice patients and their families in home and facility settings, working alongside the hospice team to deliver compassionate, high-quality care. The role involves travel to patient homes, facilities, and communities within the assigned coverage area and offers flexible scheduling.

Requirements

Current New Mexico CNA certification.
Reliable transportation and a valid driver’s license.
Ability to work independently in home settings with professionalism and compassion.
Strong communication, time-management, and interpersonal skills.
Comfort supporting patients and families during end-of-life transitions.

Nice To Haves

At least six months of direct patient care experience is preferred.
Hospice or home health experience is preferred.

Responsibilities

Provide hands-on personal care, including bathing, grooming, dressing, repositioning, and activities of daily living support.
Help maintain a safe, clean, and comfortable environment in home or facility settings.
Observe and report changes in patient condition to the RN Case Manager.
Offer emotional support to patients and families with sensitivity and respect.
Document care accurately in accordance with regulatory and organizational standards.
Communicate with the care team to support coordinated, high-quality care.
Travel to patient homes, facilities, and communities within the assigned coverage area.
