Requirements

  • Must hold current certification as a nurse aide and be in good standing on the state registry
  • Valid driver's license and reliable transportation with valid insurance
  • Willingness to travel frequently within the assigned service area
  • Ability to read, write, and communicate effectively in English
  • Ability to perform the physical demands of the role, including standing, bending, lifting, and use of transfer equipment

Nice To Haves

  • Minimum 1 year of experience as a hospice aide, home health aide, or certified nursing assistant preferred

Responsibilities

  • Travel to patient homes and facilities within the assigned service area
  • Assist patients with activities of daily living (bathing, dressing, grooming, toileting)
  • Support safe transfer, ambulation, positioning, and comfort measures
  • Assist with meals, hydration, and basic care-related homemaking
  • Take and document vital signs as directed
  • Observe and report changes in patient condition to the RN
  • Follow individualized Hospice Care Plans while adapting to assignment changes and varying care environments
  • Document care provided and communicate professionally with patients, families, facility personnel, and the care team
