Requirements
- Current certification as a Nurse Assistant (CNA) with hospice experience preferred.
- Compassionate and patient-focused attitude.
- Excellent communication and interpersonal skills.
- Ability to work effectively both independently and as part of a team.
- Basic computer skills for documentation purposes.
- Flexible availability to meet patient needs, including potential weekends or evenings.
Responsibilities
- Assist hospice patients with daily living activities including bathing, dressing, grooming, and mobility.
- Monitor and report changes in patients’ conditions to nursing staff and healthcare providers.
- Provide emotional support to patients and their families during end-of-life care.
- Maintain accurate and timely documentation of care provided.
- Ensure compliance with hospice care standards and protocols.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
