Join Our Hospice Team We are seeking a dedicated RN Case Manager to coordinate, implement, and evaluate individualized plans of care for hospice clients. In this role, you will use your clinical judgment, assessment skills, and compassionate communication to support clients and families through some of life’s most important moments.

Requirements

  • Current Wisconsin Registered Nurse license.
  • Current CPR certification.
  • One to two years of clinical nursing experience; hospice experience preferred.
  • Strong assessment, communication, organization, and documentation skills.
  • Ability to travel to client homes with a valid driver’s license, current vehicle liability insurance, and a driving record that meets St. Camillus standards.
  • A compassionate, dependable approach and a commitment to honoring each person’s dignity, comfort, and care goals.

Responsibilities

  • Perform timely initial evaluations and ongoing nursing assessments for hospice clients.
  • Develop, coordinate, and update individualized plans of care in collaboration with the interdisciplinary team.
  • Educate and support clients, families, caregivers, and team members with compassion and professionalism.
  • Monitor changes in client condition and communicate promptly with physicians and hospice leadership.
  • Maintain accurate, timely clinical documentation in accordance with agency policies and state and federal regulations.
  • Provide leadership, teaching, and support to hospice aides, licensed practical nurses, and other care team members.
