About The Position

The Hospice RN Case Manager is responsible for providing comprehensive care to patients facing a terminal illness. This includes assessing physical, functional, psychosocial, and environmental needs, developing and implementing care plans, and educating patients and families. The role involves coordinating with an interdisciplinary team, providing clinical direction to other nursing staff, and ensuring documentation meets quality standards and payer requirements. The position also requires participation in quality improvement programs, adherence to hospice philosophy, and performing on-call duties. Effective communication with patients, families, and healthcare professionals is essential.

Requirements

  • Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Current state license as a registered nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact.
  • Current driver’s license.
  • Experience as a registered nurse in a clinical care setting.
  • Must read, write and comprehend English.
  • Reliable transportation and valid auto liability insurance.

Nice To Haves

  • Hospice experience preferred.
  • Specialized skills preferred in palliation and end-of-life.

Responsibilities

  • Conduct initial and ongoing comprehensive assessments of patients' physical, functional, psychosocial, and environmental needs related to their terminal disease.
  • Apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and patient eligibility.
  • Implement, develop, and document the plan of care, ensuring quality and continuity of care, and recommending revisions as necessary.
  • Consult with and educate patients and families regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs.
  • Prioritize needs with the members of the IDG/T and provide training to other staff as needed.
  • Determine the scope and frequency of services needed based on acuity and patient/family needs.
  • Assess the caregiver's ability to meet the patient’s immediate needs upon admission and throughout care.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Apply concepts of infection control and standard precautions in patient care activities.
  • Provide clinical directions to the Hospice Aide and LPN/LVN to ensure quality and continuity of service.
  • Prepare clinical and progress notes demonstrating progress toward established goals and ensure continuity of quality patient care with appropriate documentation.
  • Inform the physician and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient and family response to care.
  • Monitor assigned cases to ensure compliance with requirements of third-party payers.
  • Participate in the Hospice sponsored inservice programs and maintain or improve competency.
  • Participate in the Hospice’s QAPI program.
  • Promote the Hospice’s philosophy and administrative policies.
  • Perform on-call responsibilities and provide on-call services to patients and their families as assigned.
  • Provide effective communication to patients, family, team members, and other health care professionals.
