About The Position

We’re hiring a Hospice Case Manager (RN) who wants meaningful patient relationships, weekday stability, and the autonomy to provide compassionate care where patients are most comfortable: their homes. You’ll step into a field-based hospice role focused on assessments, symptom management, care planning, patient and family education, and interdisciplinary coordination. You’ll have independence in the field while remaining connected to a supportive hospice team.

Requirements

• Active Georgia RN license
• Current CPR certification
• Recent nursing experience, ideally in home care or hospice
• Comfortable assessing patients and managing symptoms independently
• Effective communication with patients, families, physicians, and care team
• Valid driver’s license and reliable, insured transportation
