About The Position

We’re hiring a Hospice Case Manager (RN) who wants meaningful one-on-one patient relationships, a flexible part-time schedule, and the opportunity to provide compassionate end-of-life care while maintaining an excellent work-life balance. You’ll step into a field-based hospice role providing direct patient care in homes and care facilities throughout Easton, Lakeville, Taunton, and Rockland. You’ll independently manage your patient caseload while collaborating with an experienced interdisciplinary team dedicated to providing exceptional end-of-life care.

Requirements

• Active Massachusetts RN license
• At least 1 year of nursing experience
• Comfortable independently managing a field-based caseload
• Works well as part of an interdisciplinary healthcare team
• Comfortable traveling throughout Easton, Lakeville, Taunton, and Rockland

Nice To Haves

• Hospice, home health, ICU, or oncology experience is preferred
• Value compassionate, patient-centered end-of-life care
