Hospice Care Nurse Practitioner (NP) - Per Diem
About The Position
Lake Sunapee VNA and Hospice is seeking an experienced Nurse Practitioner (NP) to join our Hospice program in a per diem capacity. This role involves providing direct patient care visits for routine provider visits, symptom management, and Hospice Face to Face encounters. On-call rotation is required. Lake Sunapee Region VNA & Hospice provides home health care and hospice services designed to help individuals remain safe, healthy, and independent in their homes throughout Grafton, Merrimack, and Sullivan counties in New Hampshire. The ideal candidate is a Nurse Practitioner (NP) who values personalized, patient-centered care and meaningful connections with patients and families, and is either experienced in home health and hospice or interested in learning more about these specialties.
Requirements
- Current NH Nurse Practitioner (NP) license
- Minimum 2 years of health care experience
- Reliable transportation
- Valid driver’s license
- Automobile insurance
- Current physical exam
- TB testing
- Required vaccinations
Nice To Haves
- Experience in hospice and/or end of life care
- Sympathetic management
- Strong communication skills with demonstrated ability to collaborate with an interdisciplinary team
- Comfortability with educating and guiding patients, families, facilities and community in end-of-life discussions, and decision-making
- Hospice, Palliative or end-of-life care experience
Responsibilities
