About The Position

Lake Sunapee VNA and Hospice is seeking an experienced Nurse Practitioner (NP) to join our Hospice program in a per diem capacity. This role involves providing direct patient care visits for routine provider visits, symptom management, and Hospice Face to Face encounters. An on-call rotation is required. Lake Sunapee Region VNA & Hospice provides home health care and hospice services designed to help individuals remain safe, healthy, and independent in their own homes throughout Grafton, Merrimack, and Sullivan counties in New Hampshire. The ideal candidate is a Nurse Practitioner (NP) who values personalized, patient-centered care and meaningful connections with patients and families. Experience in home health and hospice is a plus, but we are also open to training those interested in learning more about these specialties. The role requires strong communication skills, the ability to collaborate with an interdisciplinary team, and comfort in educating and guiding patients, families, facilities, and the community in end-of-life discussions and decision-making.

Requirements

Current NH Nurse Practitioner (NP) license
Minimum 2 years of health care experience
Reliable transportation
Valid driver’s license
Automobile insurance
Current physical exam
TB testing
Required vaccinations

Nice To Haves

Experience in hospice and/or end of life care
Experience in sympathetic symptom management
Demonstrated ability to collaborate with an interdisciplinary team
Comfortability with educating and guiding patients, families, facilities and community in end-of-life discussions, and decision-making
Hospice, Palliative or end-of-life care experience is preferred
