About The Position

The Hospice Care Consultant (HCC) role is a field-based position responsible for representing Moments Hospice and building relationships with referral sources. The goal is to change the hospice experience by providing compassionate end-of-life care. The company is described as one of the fastest-growing hospice providers in the Midwest, South, and Mid-Atlantic, emphasizing a culture of presence, responsiveness, and patient/family-centered care. The HCC is expected to connect with referral sources, and the clinical and operations teams will follow through on promises made. Requirements

Relationship building skills.
Ability to represent something they believe in.
Drive, empathy, and follow-through.
Experience in hospice or home health sales.
Experience in medical device or pharma sales.
Experience as a clinical liaison or RN liaison.
Experience in SNF or AL business development.
Outside sales experience from other industries.
RN, nursing, social work, or chaplaincy background with a desire to move into sales.
Readiness to step into hospice sales with the right drive and heart.

Responsibilities

Represent Moments Hospice to referral sources.
Build relationships with referral sources.
Connect with referral sources and ensure follow-through from clinical and operations teams.
Drive business growth for the hospice provider.
