About The Position

This is a full-time position for a Hospice and Palliative Nurse Practitioner. The role involves providing and documenting medically necessary services, participating in patient recertification and discharge decisions, and visiting patients at home to assess hospice appropriateness and make treatment recommendations. The Nurse Practitioner will write treatment orders, work with an interdisciplinary care team, and develop treatment plans that maximize patient health potential. This includes ordering and interpreting diagnostic tests, prescribing medications, and developing education plans. The position also involves advanced care planning, consulting with physicians, building relationships with healthcare professionals and community members, referring patients to specialists, and assisting with care coordination. The role includes developing outreach programs, providing disease management education, offering clinical guidance to facility staff, performing on-call services as needed, and maintaining accurate patient records. The Nurse Practitioner will act as a clinical resource for complex cases, ensure compliance with laws and regulations, review policies with patients and obtain consent, and collaborate with the Intake department for coverage verification and reimbursement. The position also involves assisting in the development of clinical practice guidelines and adhering to ethical and professional standards, including HIPAA regulations and employee handbook policies. Participation in in-service education and maintaining a professional appearance are also required. Other duties as assigned.

Requirements

  • Board-licensed nurse practitioner, licensed to practice in New Jersey (PA license may also be required depending on territory served).
  • Hospice and Palliative Care Certification.
  • Complies with current department of Public Safety and maintain Drug Enforcement Agency registrations.
  • Knowledge of the palliative care philosophy of care and the delivery of medical care at home.
  • Knowledge of the dying process; knowledge of policies and procedures of the hospice organization.
  • Maintains a broad base of technical knowledge and skills to perform all assigned clinical/administrative duties.
  • Ability to understand and follow state regulations regarding practice agreement with physician.
  • Ability to function as a resource to patients and families during times of high stress and volatility.
  • Ability to assess patients to determine appropriateness of participation in hospice.
  • Ability to build constructive relationships with colleagues, and outside agencies, organizations, and individuals.
  • Excellent communication skills to effectively deliver information between referral source and the hospice clinical team.
  • Consistently model respect for, and accountability to, the public, patients/families, and co-workers.
  • Communication and actions reflect care, courtesy, honesty, and professionalism.
  • Conveys a professional and positive image and attitude regarding the hospice organization.
  • Experienced in the practice of family practice, oncology or hospital practice.

Responsibilities

  • Provides and documents medically necessary services in accordance with physician/NP protocol.
  • Participates in the decision to recertify and discharge patients to/from hospice & palliative care.
  • Visits patients in home to assess recertification appropriateness for hospice and make treatment recommendations.
  • Writes treatment orders and documents as appropriate.
  • Works with the interdisciplinary care team to ensure that the medical needs of the client are met.
  • Develops the appropriate treatment plan that maximizes the health potential of the patient (i.e., ordering and interpretation of diagnostic tests, identifying appropriate pharmacologic agents, identifying appropriate non-pharmacologic interventions, developing an education plan as appropriate, interpretation of laboratory and radiology tests, ordering of durable medical equipment).
  • Performs advanced care planning for the maximization of continuity of care.
  • Consults with collaborative physicians, consulting practitioners, primary care physician and other specialist physicians.
  • Establishes relationships with existing hospital-based hospice palliative care teams, partner organizations, medical directors, hospital physicians, community physicians, nurses and other team members as well as community members to build a culture of collaboration and clinically integrated care across the continuum.
  • In collaboration with PCP, refers patients to medical specialists and/or refers patients to other healthcare services as medically indicated.
  • Assists in all facets of care coordination for referrals.
  • In association with Marketing department, develops outreach programs for community agencies and health care setting.
  • Participates in outreach presentations and education to interested groups.
  • Provides disease management instruction and education to patients and their families.
  • Provides clinical guidance and education to facility staff related to patient care issues, assessments and interventions.
  • Performs reasonable on-call services as Needed.
  • Participates with care setting’s interdisciplinary team as appropriate.
  • Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided, and support of appropriate medical uses.
  • Acts as a clinical resource to coordinate complex cases for safe and appropriate transitions to other care settings.
  • Complies with applicable laws and regulations with respect to Delegation Protocol Agreements, prescriptive authority, and NP scope of practice.
  • Reviews KAQ policies and services with referred patients and/or family caregivers or authorized patient representative and obtains consent for medical care.
  • Works collaboratively with Intake department in verification of coverage/payment and initiates reimbursement for services rendered.
  • Assists in the development and review of clinical practice guidelines/standards in support of quality care, and the preparation of associated policies and procedures.
  • Acts with integrity, compassion, and customer focus to ensure quality care.
  • Adheres to employment policies as described in the employee handbook; maintains and respects confidentiality; adheres to all HIPAA regulations.
  • Participates in all required in service education programs and meetings.
  • Demonstrates compliance with the dress code policy by appearing well groomed, wearing ID badge while on duty and maintaining a professional appearance.
