As a Capstone NP, you will provide advanced practice nursing management of the hospice patient's terminal condition. This role requires performing face-to-face encounters in patients' homes and senior living communities for recertification reviews. You will work independently while collaborating with physicians, nurses, social workers, and chaplains to deliver end-of-life care. This is a full-time position, Monday-Friday, with autonomous work. To ensure consistent patient satisfaction, NPs should reside in Cobb County or the NW area of Metro Atlanta for reasonable commute times.

Requirements

  • Completion of an accredited NP or PA program.
  • Currently licensed as an advanced practice nurse in the State of Georgia by the Georgia Board of Nursing and certified as a NP by the American Nurses Credentialing (ANCC) or equivalent body or certified as a PA by the National Commission on Certification of Physician Assistants (NCCPA).
  • A minimum of 1 year experience on a hospice care team or in another setting managing the care of terminally ill patients.
  • Knowledge about the medical aspects of caring for terminally ill patients and the psychosocial and spiritual impact of terminal disease on patients and families.
  • Understanding of Medicare guidelines for hospice.
  • Ability to work independently and collaborate with IDG team.
  • Emotional intelligence and ability to hold interpersonal relationships.
  • Experience using a Healthcare EMR.
  • Experience with Microsoft products to include Outlook, Teams, Excel or Word.
  • Make accurate and at time quick judgements.
  • Ability to respond appropriately to crisis outside of a hospital setting.
  • Acceptance and adaptability to different social, racial, cultural and religious modes.

Responsibilities

  • Performs face to face encounters on hospice patients.
  • Assists in evaluation of patient medical eligibility of Hospice services in accordance with the Hospice program policy. This includes face to face medical assessment or review of hospice eligibility worksheet with standard Palliative Performance Scale information upon each recertification.
  • Review clinical documentation for patients related to the disease process, drug regime, and prognosis upon admission and throughout hospice care.
  • Available for consultations with the hospice team concerning patient care situations and quality improvement.
  • Consults with a patient's primary physician as requested.
  • Reviews medication utilization and follows accepted standards of practice.
  • Ensures that each patient is assessed using a generally accepted life expectancy prognostic scales to determine continued eligibility for hospice admission in accordance with Medicare guidelines. Documentation must include a written certification by the Medical Director or the patient's attending physician.
  • Make visits to hospice patients as indicated.
  • Ensure that all care is provided with respect for patient rights and goals of care.
  • Report all grievances and complaints made by patients and families to the appropriate person(s).
  • Report all allegations of patient abuse and/or misappropriation of patient property.
  • Reports all accidents and incidents.
