About The Position

As a hospice aide, you provide compassionate, hands-on care to help patients remain comfortable and maintain dignity throughout their end-of-life journey. You support personal care, mobility, activities of daily living, nutrition, and light homemaking related to patient care. You help patients maintain independence and take part in meaningful activities, and observe and report changes in their condition to the registered nurse.

You will care for hospice patients wherever they call home, including private residences and assisted living or skilled nursing facilities. The role involves travel within an assigned service area and is performed under the supervision of a registered nurse as part of an interdisciplinary hospice team.

Requirements

• Approved hospice aide, home health aide, or nurse aide training and competency.
• Ability to travel as required by the role.
• State-specific certification requirements apply: AL: CNA; CA: CHHA; CT: Homemaker-Home Health Aide; DC, DE, FL, GA, PA, TX, VA, WI: CNA, HHA, or HA; IL: HHA or NA; KS, MO: Home Health Aide; OH: State Tested Nurse Aide; NJ: Homemaker-Home Health Aide.

• One year of direct patient care experience preferred.

Responsibilities

• Assist patients with personal care, mobility, and activities of daily living.
• Support nutrition, including simple meal preparation and feeding.
• Provide light homemaking support related to the patient’s care.
• Help patients maintain independence and participate in activities meaningful to them.
• Observe and report changes in a patient’s condition to the registered nurse.
• Travel within the assigned service area to provide care in patients’ homes or facilities.
