About The Position

The Hospice Aide provides personal care to clients according to prescribed policies and procedures, including applicable state, federal, and accreditation regulations. Under the general supervision of the RN Case Manager, the aide performs various services to meet the client’s personal needs and promote comfort. This role involves observing, recording, and reporting client care, symptoms, and emotional/physical status, while offering compassionate support to clients and their families.

Requirements

• Minimum of 12 months as an aide in a facility or home health/hospice agency or acute care setting.
• Home Health Aide Certification training.

Nice To Haves

• STNA strongly preferred.
• Hospice experience preferred.

Responsibilities

• Performing and documenting all duties assigned by the RN Case Manager.
• Observing, recording, and reporting the client’s care, symptoms, emotional and physical status.
• Providing personal care including bathing, back rubs, oral hygiene, hair care, skin care, assisting client with dressing and undressing, and assisting the patient with toileting activities.
• Processing client’s laundry, as appropriate, changing bed linen and keeping client’s living area clean and orderly, as appropriate.
• Planning and preparing nutritious meals and assisting in feeding the client, if necessary.
• Taking vital signs when ordered.
• Assisting in ambulation and transfers as appropriate to plan of care.
• Performing range of motion and other simple procedures as an extension of therapy service as ordered.
• Meeting safety needs of clients and using equipment safely and properly.
