About The Position

Heart to Heart Hospice provides compassionate care to patients with life-limiting illnesses and their loved ones. The Hospice Aide - CNA provides direct patient care under the direction of an RN and in accordance with the Hospice Aide/Homemaker Plan of Care. The role supports safe patient transfers and ambulation, communicates and documents relevant changes and care, and promotes respectful, supportive communication with patients, families, and colleagues. The position also requires timely documentation and compliance with applicable laws, Medicare regulations, and agency policies.

Requirements

High school diploma preferred or GED.
Unencumbered certification according to the applicable Nurse Aide Registry.
Current CNA license.
At least one year of experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility.
Demonstrated interest in the welfare of ill and elderly people and a commitment to providing compassionate care.
High integrity, including maintaining confidential information.
Current driver's license, reliable transportation, and ongoing valid and current auto liability insurance.

Responsibilities

Provide direct patient care under the direction of the RN and according to the Hospice Aide/Homemaker Plan of Care.
Assist patients with safe transfers and ambulation according to agency policy.
Report and communicate changes and document pertinent information and care provided to ensure continuity of care.
Practice accepted infection control principles.
Promote positive, supportive, and respectful communication with patients, families, and other employees.
Submit all documentation in a timely manner.
Perform other duties as required.
Ensure overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
