About The Position

Tapestry Hospice & Palliative Care is seeking a compassionate and dedicated Hospice Aide / Certified Nursing Assistant (CNA) to join their family-owned and operated team. As a vital member of the Interdisciplinary Team, the CNA will be the heart, eyes, and key communicator of patient needs, following a personalized care plan and collaborating with the RN Case Manager and other team members to provide high-quality care with compassion and excellence to patients and their loved ones.

Requirements

• Current CNA License in the State of Georgia
• Current CPR & First Aid Certification
• 1–2 years of hospice experience
• Utmost compassion and integrity
• Valid driver’s license, insurance, and reliable vehicle
• Excellent time management skills

Nice To Haves

• Experience in assisted living, home health, or long‑term care

Responsibilities

• Follow a carefully planned, personalized care plan for each assigned patient.
• Work closely with the RN Case Manager to ensure care is delivered with excellence.
• Collaborate with all members of the Interdisciplinary Team.
• Provide compassionate, professional, and high-quality care to patients and their loved ones.
