In the role of Hospice Admission RN, you will also be responsible for initiating the hospice plan of care and ensuring clear, timely communication in accordance with established guidelines. Requirements

  • Current RN license (BSN preferred)
  • Minimum 1 year of nursing experience (required in California)
  • Valid driver’s license and reliable transportation
  • Strong computer skills and a commitment to ethical, mission-driven care

Nice To Haves

  • Home healthcare experience preferred

Responsibilities

  • Provide skilled nursing care in accordance with physician orders and organizational policies
  • Perform comprehensive assessments and develop individualized care plans
  • Collaborate with physicians, therapists, social workers, and home health aides
  • Supervise and support LPNs and Home Health Aides
  • Ensure compliance with regulatory standards and contribute to quality improvement initiatives
  • Promote patient satisfaction and positive health outcomes
  • Initiating the hospice plan of care and ensuring clear, timely communication in accordance with established guidelines.
